In order for Accutech Restoration to consider you for work with our clients, we require the following items to be submitted to our main office:
- Current liability insurance certificate with Accutech noted as the certificate holder.
- Current workers compensation or workers compensation exemption certificate.
- Signed W-9
- Signed subcontractor agreement.
