In order for Accutech Restoration to consider you for work with our clients, we require the following items to be submitted to our main office:

• Current liability insurance certificate with Accutech noted as the certificate holder.
• Current workers compensation or workers compensation exemption certificate.
• Signed W-9
• Signed subcontractor agreement.
